I've learned a couple of replacements for the "__" in German writing, like "sz" and "hs". These replacements look somewhat similar in older fonts, and their ligatures may be the root of the "__". The "sz" also may be a (wrong) decomposition of the old and phonetically closest "hs" ligature, resulting from an attempt to mimic its look in an different font.
